Patient Guide
Upon Admission
|
We intend to make you feel as comfortable as possible as soon as you arrive at Manila Doctors Hospital the Admission clerk may ask for the following:
- Doctor's Order
- Room rates and Available rooms
- Should decide to use Health Cards or Company Account, please proceed to the Credit and Collection Department for Approval.
- Your senior citizen card and booklet.
Admission Form
Please fill up the Admission Form with the necessary information, ensuring the completeness and accuracy as much as possible. This is a vital procedure and we appreciate your cooperation.
Contract of Admission
Please read thoroughly before signing the contract of admission. It contains pertinent information regarding your stay and privileges as a patient, as well as the hospital’s prerogatives.
Admission Kit
The hospital provides each admitted patient an admission kit, free of charge that contains a clinical thermometer, a medicine glass, a calibrated drinking glass, fork and spoon and tissue paper.
Clothes and Linen
The hospital supplies all your hospital clothes and linen. Please bring with you clothes that are necessary for check out only.
Deposits
A deposit equivalent to the rate of the room for at least five (5) days is required on admission. Credit and Collection furnish patients with their Statement of Account every two (2) days thereafter to update the patience of their current account. You may also request for your Statement of Account at the Billing Section, Ground Floor.
For emergency cases, a deposit will not be required. MDH will admit the patients to the lowest bed available.
Firearms and Deadly Weapons
Firearms and deadly weapons are strictly prohibited within the hospital. Deposit them with the Security Guard at the Lobby in exchange of a firearms report.
No Smoking
The whole hospital premise is a smoke free zone.
Health Maintenance Organization (HMO) and Corporate Accounts
Patients of companies, who are accredited with the hospital, shall submit a Letter of Authorization (LOA) from the HMO. Otherwise, the bill will be the personal account of the patient.
Philhealth
Kindly inform the Admitting Section upon admission if you are a Phil-Health member. Forms can be secured from the Accounting Department. Submit your completed papers before discharge so you can take advantage of our benefits. For further inquiries please call Phil-Health Section.
Valuables
We encourage you to utilize the Safety Deposit Box for valuables and important documents. THIS SERVICE IS FREE. Contact the Cashier Department for more details
